Der Nervus oculomotorius entspringt im vorderen Anteil des Mittelhirns und verlässt dieses in der Fossa interpeduncularis.Seine Hirnnervenkerne sind der Ncl. nervi oculomotorii (somatomotorisch) und der Ncl. accessorius nervi oculomotorii (Edinger-Westphal-Kern) (parasympathisch) sowie der Kern von Perlia für die Akkommodation. Die Behandlung einer Schädigung des Nervus oculomotorius erfolgt durch einen Neurologen. Die Heilungschancen richten sich nach der Ursache der Erkrankung. So gilt die Prognose als günstiger, wenn der Patient unter Durchblutungsstörungen leidet. Bei Aneurysmen oder Tumoren ist dagegen mit einem ungünstigen Verlauf zu rechnen.
